Physician - Family Practice: Aurora - Full-time/Part-time Health Center Aurora Clinic Hours - M-Th: 8:00am – 8:00pm, Qualifications: Knowledge/Skills: Must have the knowledge, skills and ability to provide care to Family Practice patients. Must be able to be on rotation for hospital rounds and take calls. Type/Years Experience: Must have 1-2 years experience as a Family Practice Physician. Job Summary: Nurse Midwife: Aurora - Full-time Health Center Work Hours: Full-time Qualifications: Education: Graduate of accredited advanced practice program with an Illinois Nurse degree. Knowledge/Skills: RN/NP skills necessary to perform physical exams and manage/supervise pre-natal patients. Bilingual in Spanish/English preferred. Type/Years Experience: Greater than 1 year experience as midwife. Job Summary: Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronic disease health services at the VNA Health Center clinic sites (on and off site clinics). OB/GYN: Aurora & Elgin - Full-time/Part-time Health Center Aurora Clinic Hours - M-Th: 8:00am – 8:00pm, Elgin Clinic Hours - M, T, W, F: 8:00am – 4:30pm, Qualifications: Knowledge/Skills: Must have the knowledge, skills and ability to provide care to obstetrical patients. Must be able to be on rotation for hospital rounds and take calls. Type/Years Experience: Must have 1-2 years experience as an Obstetrician. Job Summary: Occupational Therapist - Aurora & Elgin: Full-time/Part-time Home Health Work Hours: M-F with 4 day work week as scheduled. Saturday PRN. Qualifications: Education: Occupational Therapist licensed in Illinois required and NBCOT. Knowledge/Skills: Ability to provide occupational therapy rehabilitation services and tasks involving physical activity, which includes heavy lifting and extensive bending and standing. Type/Years Experience: Minimum of one year experience in a laboratory setting. Job Summary: Under the guidance of the Team Leader Nurse, performs laboratory screening procedures for Health Center clinic clients as assigned. The Phlebotomist also performs other tasks as assigned to assist in well functioning of Health Center activities. Registered Dietitian: Aurora - Full-time (2 locations)/Part-time Aurora Clinic & Part-time VNA Tomcat Health Center at Aurora East High School Work Hours: M-F 8-4:30 and evenings and Saturdays as needed. 1 FTE (.6 Tomcat and .4 WIC Clinic) Qualifications: Education: Bachelor’s degree, completed at least 900 hours of supervised dietetics practice under the supervision of a registered dietitian or nutrition professional, and registered/licensed dietitian. Knowledge/Skills: Bilingual in Spanish/English a plus. Must be adolescent friendly and act as a role model for patients seen. Type/Years Experience: At least one year of clinical experience, WIC experience a plus. Job Summary: A nutrition professional responsible for providing nutrition assessment, planning, counseling and education for Tomcat Health Center patients and WIC clinic patients. Bilingual CNA: Elgin - Full-time Elgin Clinic Hours - M, T, W, F: 8:00am – 4:30pm, Qualifications: Education: Certificate of completion from accepted training program as required by State Law. Registration on the Dept. Nurse Aide Registry. Knowledge/Skills: Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing. Job Summary: Under the guidance of the Director of VNA Health Center and Staff Nurses, performs basic screening procedures for Child Health, Women’s Wellness and WIC clinic clients as assigned. Qualifications: Education: Board Certified Physician in Pediatrics and Licensed in Illinois required. Job Summary: Support end-users with set-up, training and maintenance of computers and phone systems throughout the organization. Educate staff in productive use of various software products.
